Therapeutic Action
Drugs used in diabetes, combinations of oral hypoglycemic agents.
Indications
In type 2 diabetes: as an adjunct to diet and exercise to improve glycemic control in patients who have not achieved adequate control with the maximum tolerated dose of metformin or vildagliptin monotherapy, or who are already being treated with the combination of vildagliptin and metformin in separate tablets. Indicated in combination with a sulfonylurea. As an add-on therapy to insulin.
Dosage and Administration
Individualize the use of the hypoglycemic agent based on efficacy and tolerability. Do not exceed the maximum daily dose of 100 mg of vildagliptin. Metformin: preferably, the maximum daily dose should be divided into 2 or 3 doses. In patients with glomerular filtration rate <60 mL/min, factors that may increase the risk of lactic acidosis should be evaluated. Contraindicated in patients with glomerular filtration rate <30 mL/min.
Contraindications
- Hypersensitivity to the active ingredients or excipients. • Severe renal impairment (glomerular filtration rate <30 mL/min). • Acute or chronic metabolic acidosis, such as lactic acidosis or diabetic ketoacidosis. • Acute conditions that may alter renal function, such as: -Dehydration, -Severe infection, -Shock, -Intravascular administration of iodinated contrast agents. • Diseases that may cause tissue hypoxia, such as: -Congestive heart failure requiring drug treatment, or respiratory failure. -Myocardial infarction, -Shock. • Hepatic impairment. • Acute alcohol intoxication, alcoholism. • Breastfeeding.
